Food/Beverage Sample Permit Application -- Will you be Serving Food or Beverage Samples? This is for exhibitors who will be handing out samples (pre-packaged or unpackaged) and/or preparing food at their booth. Exhibitors MUST fill out the application to obtain a health permit for their booth.
